Fees

• If you use your health insurance:

One hour session fee                     $85
I am a contracted or network provider for First Choice, Group Health and LifeSynch.  If you have one of these plans, you pay me a copay amount and I will bill your insurance company directly.  Otherwise, please find out if your insurance policy covers mental health benefits by an out-of-network provider and also if there is a deductible.   For Premera, Regence and other major insurance companies, I can submit claims electronically on your behalf.  Otherwise, I will provide a form with your diagnosis and the payment information for you to submit to your insurance company after you pay me a full fee.  It is your responsibility to get reimbursed from your insurance company.

• If you do not use your health insurance, you get a discount for reduced administration cost.

One hour session fee                     $75

• Payments can be made in cash, check or credit cards (VISA, Master Card and Discover).

 

Location and Directions

From North                       

• Take I-5 southbound to Exit 169 for  50th St. 
• Turn LEFT onto 50th St.
• Turn RIGHT at the second traffic lights onto 9th Ave. NE
• My office building is on the third block on your left, West University Business Center where Metro Cinema is located.  Take an elevator to the third floor and ask a receptionist for my office.

From South

• Take I-5 northbound to Exit 169 for NE 45th St/University of Washington.
• Turn Right onto 45th St.
• Turn LEFT onto 9th Ave. NE (Look for Metro Cinema sign).
• My office is the first building on the right, West University Business Center where Metro Cinema is located. Take an elevator to the third floor and ask a receptionist for my office.

Parking

60-minutes free parking is available. Park at the north end of DPS parking lot across from Metro movie theater between stalls 1 and 38 or on the top floor of Trader Joe's between stalls 67 and 145. Trader Joe's is located on Roosevelt Way and 45th St. DO NOT PAY FOR PARKING.

Remember your stall number. At the reception desk on the third floor of West University Business Center (4500 9th Ave. NE), sign in with your stall number and license plate number.

Street parking is available until 6pm.

Several Metro bus and Sound Transit bus stops are nearby with many routes.
